Looks like a Chestnut Chum or Doe model, built in the 1960s.

I am more than happy to have your view. I have visited the Dragonfly site on many occasions over the years pursuing identifications. In this case the identification numbers on the stem seem to offer more puzzle than light. It is a great pleasure to have such an immediate and authoritative response.
