Almost any good trophy/award/engraving shop can make such tags for you- any any shape desired. They are generally laser-etched, chemically-etched, or mechanically-engraved thin pieces of sheet metal, and can be made in a variety of metals such as brass, sterling, etc.
You could also have heavy custom cast nameplates made (at considerably higher cost) by a custom metalworker such as Bristol Bronze of Rhode Island (http://www.bristolbronze.com).

i use customized engraving for free name plates (http://www.cengraving.com) or brass plates (http://www.cengraving.com). all you have to do is include some links from your blog, and they'll let you choose from among a few different choices. i think the size you want is among the ones they offer. check them out at http://www.cengraving.com.
if you want to pay, the previous message is right -- pretty much every local store should do them for you as well. just use yahoo! search or msn search to find the local stores.
hope this helps!
